For patients who have previously established care and are transitioning to Selah Health.

This visit includes:

• A comprehensive review of current health status and medical history

• Medication and Supplement reconciliation

• Evaluation of any new concerns or changes since your last visit

• An updated, personalized treatment plan

• Guidance on selecting the Selah Health membership that best aligns with your current healthcare needs and goals.

Your membership includes:

• Two follow-up visits each year with Megan Gietzen DNP, FNP-C

• One annual Selah Biomarkers Panel to establish a baseline and monitor your overall health and wellness

• Secure patient messaging with responses within 48 business hours for non-urgent questions, guidance, and follow-up support.
